	At-Risk Extension Medical Checklist This checklist is required for At-Risk extension requests and must be completed by a medical doctor.
	Automatic Disbursement Request Form
	Annual Occupancy Certification Form As a HUD requirement of a Reverse Mortgage, the Annual Occupancy Certification Form will ensure the loan meets HUD's expectations of annual occupancy certification. Note: For New York Co-op properties, please contact us to request an Annual Occupancy Certification Form be mailed.
	Entitled Non-Borrowing Spouse (eNBS) Annual Certification - Borrower HUD requires the borrower to certify annually that the eNBS remains married to the borrower and continues to occupy the property. This form should be completed by the borrower.
	Entitled Non-Borrowing Spouse (eNBS) Annual Certification - Spouse HUD requires the spouse to certify annually that the eNBS was married to the borrower until the time of their death. This form should be completed by the spouse when the borrower is deceased.
	Estate Planning Questionnaire This form should be completed to ensure we have accurate contact information for the estate in the event of a death. The questionnaire is not required, but will help identify who to communicate with about the property and/or act on behalf of the estate.
	Insurance Authorization This form is used to provide your authorization to communicate with the insurance agent/carrier to obtain insurance documentation relative to the servicing of the account.

	Loss Mitigation Information Packet
	Mortgagee Optional Election (MOE) Information Sheet This form is used to identify a spouse that may be eligible to be reviewed for the MOE (NBS) program and remain in the property after the death of the last remaining Home Equity Conversion Mortgage (HECM) borrower.
